In recent years, the Ministry of Health, Welfare and Sport focused on patient safety in healthcare. However, with the exception of the guideline for medication transfer, national projects were primarily sector-related, as a result of which lessons learned from incidents and other patient safety initiatives were mainly limited to the relevant sector itself.
Hardly any tools for improvement of patient safety have been developed yet for the interface between hospital care and primary care, which sees frequent healthcare transfer with what is probably an intrinsically increased risk of mistakes.
