This thesis enhances palliative care by validating and implementing the USD-4D, a PROM addressing physical, psychological, social, and spiritual symptoms. A socio-spiritual approach is proposed to better understand patient needs. The USD-4D showed strong content validity in studies with patients and healthcare providers. Meaningful use requires a respectful, personalized approach and integration into care. While benefits include better communication and holistic dialogue, barriers like time pressure and discomfort with existential themes remain. The USD-4D fosters compassionate, proactive, patient-centered care.
